The only conditions from the utility side are that the item has to be set as "Post-to-Web" and the items have to be associated with a category.

On the ZenCart side, there are options to hide items without a zero or negative on-hand quantity, so it could be being suppressed on that side.  Check the inventory maintenance function on the admin side of the cart to see if the item is there.

Does the category have to exist on the zencart side? or does it automatically creates it if it does not exist?

That would do it. The utility adds some fields to the standard tables, and if those custom fields are missing the insert and update queries will fail. Have your database administrator grant alter rights to the dbm user and then run the import again. That should allow the items to be created.
